Manager Hospice Patient Care - Full Time Exempt - Days - 8hr

Job Summary

As a key member of the Hospice service line, the Hospice Patient Care Manager will work under the direction of the Hospice Administrator and will work in coordination with the clinical care team to support and direct, as appropriate, the delivery of care for all patients. In addition, the Hospice Patient Care Manager will also manage the day-to-day operations of a ten-bed hospice facility inpatient unit. Provides comprehensive clinical management and functions as liaison between the patient/family and the clinical care team. Contributes to and participates in survey readiness as well as the evaluation and development of service line protocols, standards of care, policy and procedures as guided by evidence-based research literature reviews and clinical practice guidelines. An integral member of the program leadership team, this person works with the development and implementation of quality improvement initiatives. Must have a sold understanding of regulatory compliance as well as being familiar with the Joint Commission requirements for hospice and skilled nursing facility regulations. Participates in the development and implementation of ongoing educational activities maintain excellence in practice. Works collaboratively with other ancillary and ambulatory care services as well as medical staff to promote continuity of care. Serves as the identified Director of Nursing for the inpatient unit and the Director of Patient Care Services per regulations.

Job Requirements

Minimum Education Requirement:

BSN preferred.

Minimum Experience Requirement:

Two years of Hospice or Home Health experience in a Medicare certified agency. At least one year of case management experience preferred. Excellent customer service skills required.

Minimum License Requirement:

Current CA RN license. BLS. Valid drivers license, current liability insurance required.

Experience with SNF requirements preferred.
