Job Summary
Coordinates the care and services of selected patient populations. Promotes effective utilization and monitoring of health care resources. Interacts with the Emergency Department and/or Registration Department and acts as an overseer at the time of admission to protect the interest of the patient, the hospital and the physician.

Minimum Education Requirement:
Minimum Experience Requirement:
Two years recent acute clinical experience. Experience with discharge. Planning, utilization management and critical pathways preferred. Effective written, verbal and time management skills. Ability to work independently as well as establish relationships and function within a team. Knowledge of medical information systems. Ability to use a computer and make notations. Excellent customer service skills required.
Minimum License Requirement:
Current California LVN License. BLS. Other certification in Case Management preferred.
Must be available to work every other weekend.
