Job Summary
The Stroke Nurse Navigator (NN) will participate in planning, organizing, and evaluation of patient care within the stroke program under the guidance and supervision of the Stroke Program Manager and the Medical Director. This position supports the Stroke team through collaboration with physicians, nursing and ancillary medical staff ensuring assessments, treatments, diagnostic work-up is timely, appropriate and congruent with current stroke care guidelines. The NN will coordinate care, serving as a liaison between patient, hospital staff, and physicians. This position’s primary focus will be collaboration with the interdisciplinary healthcare team to provide a seamless, well organized in-patient hospital stay as well as an organized transition to out-patient resources.

Job Requirements

a. Minimum Education Requirement :
Bachelors of Science in Nursing within 1 year of hire/transfer.

b. Minimum Experience Requirement :
Minimum 2 years nursing experience; emergency room, critical care or neuro-interventional radiology experience preferred. Experience to demonstrate the following: 1.) ability /skill in patient and staff teaching 2.) Ability to use electronic based medical records and Microsoft Office applications.

c. Minimum License Requirement :
Current California R.N. license. Maintains NIHSS, BLS, and ACLS certification. Annual 8 hours of stroke education required for those working at QVH
